MyCareBridge is a digital neurodevelopment referral and pathway management platform that connects families, education and clinical teams to improve referral quality, communication and pathway efficiency throughout ADHD and autism assessment pathways.

MyCareBridge is a digital platform designed to support neurodevelopment pathways by improving how referrals, information sharing and communication are managed between families, educational settings and healthcare providers.

The platform addresses common challenges experienced across ADHD and autism pathways, including incomplete referrals, fragmented communication, administrative burden and limited visibility for families during extended waiting periods. By providing a single digital environment, MyCareBridge enables referral information to be collected in a structured format, with contributions from education professionals and families before clinical review begins.

Throughout the pathway, families can securely upload supporting information, receive updates on referral progress and communicate with the clinical team through the platform. Clinicians and administrative teams benefit from more complete referral information, configurable triage workflows and dashboards that provide visibility of referrals awaiting review. Automated notifications and milestone tracking help reduce unnecessary enquiries while keeping families informed.

MyCareBridge supports organisations delivering neurodevelopment services by improving referral quality, reducing avoidable administration and creating greater transparency across the pathway. The platform is designed to fit alongside existing clinical processes rather than replacing clinical decision making.

A key differentiator is its emphasis on collaborative working between education, families and healthcare professionals from the earliest stage of the pathway, helping ensure clinicians receive more complete information while giving families greater involvement and visibility throughout their journey.

Core Functions

Digital Referral Management

MyCareBridge digitises neurodevelopment referrals using structured electronic forms completed by education professionals and families. Standardised information improves referral quality, reduces incomplete submissions and supports more efficient administrative processing before clinical review.

Collaborative Information Gathering

The platform allows parents, carers and education providers to contribute relevant documentation and supporting evidence within one secure environment. This provides clinicians with a more comprehensive picture before assessment while reducing duplicated requests for information

Configurable Clinical Triage

Clinical teams can review referrals using configurable workflows, including the ability to identify incomplete referrals or referrals requiring prioritisation. This supports consistent decision-making while improving visibility across waiting lists.

Family Communication and Updates

Families receive milestone updates and secure communications throughout the pathway via a smartphone app. Providing greater transparency reduces uncertainty during long waiting periods and helps decrease routine status enquiries received by services. This also includes a built-in message and task assignment module.

Case Management Dashboards

Operational and clinical teams have access to dashboards showing referral status, workflow progress and outstanding actions. This supports service management, workload monitoring and operational oversight across neurodevelopment pathways.

Resource Library

The parent focused smartphone application comes with the ability to publish a dedicated and detailed resource library for parents to reference during lengthy wait times. This can be front loaded by a service and be as extensive as required.

Clinical Validation and Evidence

MyCareBridge has established an academic partnership with the University of Essex and has been deployed within the Mid and South Essex Community Collaborative, including NELFT, EPUT and Provide Community. The organisation reports evidence relating to service efficiency gains, waiting list reductions, increased capacity, time savings, patient outcomes and user satisfaction.

The platform is currently deployed across three NHS organisations within the Mid and South Essex region, supporting neurodevelopment pathway delivery through digital referral management, communication and pathway oversight tools.

Integration and Compatibility

Although we do not currently integrate with any other platforms, we are open to developing full EPR integration in the future.

Service User Design / Involvement

Co-Production Approach

MyCareBridge was developed in collaboration with clinicians, administrative teams and patients involved within the Mid and South Essex Community Collaborative. Their direct involvement helped shape the platform's functionality, workflows and overall user experience.

Feedback Received

Feedback was gathered throughout the development process, including reviews of the application's design, usability and overall user experience. The platform's questionnaires and pathway structures were also informed by real-world clinical and administrative processes used by participating services.

Changes Made as a Result

Insights gathered during development led to multiple design iterations and refinements. These changes were implemented to improve usability and ensure the platform met the needs of the different stakeholders involved across neurodevelopment pathways.

Ongoing Involvement

Engagement with current clients continues to inform the development roadmap for MyCareBridge. Regular discussions with users help identify opportunities for further enhancement and future platform development.

We are keen to understand how people with lived experience have influenced the design and development of your innovation.
